The Club has limited financial resources available for those who need aid with club fees. This form must be completed and submitted to be considered for financial aid. Applications are considered on a case-by-case basis, and all decisions are in the sole discretion of the Club. All families receiving financial aid will be asked to contribute volunteer time to club and team functions and to fund-raisers to pay for any financial aid.
If eligible for free/reduced school lunch, food stamps, or similar program, please attach a confirmation of that eligibility. You must also submit a copy of your most recent tax returns.
All players, directly or through others, must pay all team and training fees including: Tournament fees; Coach’s travel expenses to tournaments, state registration and referee fees, tournament fees, indoor season or practice facility rentals, and other items deemed necessary by the Team Coach.
A player who receives financial assistance, who leaves before the seasonal year is complete, must pay back any financial assistance before transfer/release paperwork will be signed.
PLEASE NOTE: TO COMPLETE THIS APPLICATION, A COPY OF YOUR MOST RECENT TAX RETURNS MUST BE UPLOADED. INCLUDE ADDITIONAL DOCUMENTATION TO CONFIRM ELIGIBILITY IN SCHOOL FREE LUNCH, FOOD STAMPS, ETC PROGRAMS. BLACK OUT ANY SOCIAL SECURITY NUMBERS WHEN SHARING INFORMATION.
